tomas®-RPE eyelet

L'ancoraggio scheletrico secondo il concetto Pin-Last©.

Con l'ausilio del tomas®-RPE eyelet viene ulteriormente semplificata la realizzazione dell'ancoraggio scheletrico ibrido RPE o MARPE (Mini screw Assisted Rapid Palatal Expansion), nonché l'inserzione dei tomas®-pin. Secondo il concetto Pin-Last© viene dapprima pianificata la posizione delle mini viti, poi realizzata l'apparecchiatura RPE e infine inseriti i tomas®-pin con la guida dei tomas®-RPE eyelet.

Il mini-impianto palatale

Con l'ausilio del tomas® PI (impianto palatale) si crea una possibilità di ancoraggio temporaneo assoluto per il trattamento ortodontico. Il tomas® PI presenta spire autofilettanti con superfici sabbiate e mordenzate. Il tomas® PI-abutment multiple set (REF 303-400-00), contenuto in una confezione sterile, funge da tappo di chiusura dell'impianto durante la fase di guarigione.

Questo abutment e altri abutment tomas® PI vengono utilizzati successivamente per l'accoppiamento del tomas® PI con l‘apparecchiatura ortodontica.

I due filmati per il tomas®-pin EP e il tomas®-pin SD mostrano quanto sia rapido e semplice l'inserzione di entrambi i tomas®-pin. Grazie alle spire autofresanti non è necessaria una perforazione preventiva. A seconda della densità ossea, si consiglia solo la perforazione dell'osso corticale. Già dopo solo mezzo giro, la punta della vite penetra nell'osso senza troppa fatica.

TADs - Basic Biomechanics

Over the last decade TAD applications have evolved into sophisticated treatment solutions for both orthodontic and orthopedic corrections. Everywhere lectures talk about advanced concepts for TADs while basic concepts appear to be taken for granted, and are no longer explained. It is exactly these basics that foster successful TAD use however, and hence form the foundation for everyone that wants to use orthodontic miniscrews. This lecture is aimed at all the new orthodontists that have graduated over the last decade and need to learn the basics, and at all the other orthodontists that need some brushing up on the basics.  This lecture also is the first of two introductory modules for attendees of the Dentaurum tomas® TAD Conference on November 8 and 9, 2024 in Las Vegas.

Skelettale Verankerung: Grundlagen & Basics

Die Gebiss- und Zahnentwicklung ist ein komplexes aber auch zugleich spannendes Thema. Abweichungen und komplexe funktionelle Störungen im Ablauf können zu Fehlbildungen, skelettalen und dentalen Wachstumsstörungen führen. Die Therapie konzentriert sich dann meist nicht mehr nur auf die Korrektur von einfachen Zahnfehlstellungen im Sinne der klassischen Orthodontie, sondern erfordert nicht selten die Einbeziehung von innovativen Behandlungsansätzen, modernen Arbeitstechniken und interdisziplinärer Kooperationen mit anderen Fachgebieten.

„Zuhören“, „Palpation“, „Sehen“ und „Verstehen“ sind grundlegende Pfeiler einer ausgewogenen Diagnostik und treten angesichts einer ständig zunehmenden Technologisierung nicht selten in den Hintergrund.

Der Kurs geht zunächst in kompakter Form und in chronologischer Abfolge auf die einzelnen Phasen der physiologischen Gebiss- und Zahnentwicklung ein und greift aus dem Gesamtspektrum vielfältiger Dysgnathien Schwerpunkte heraus. Es werden anhand von zahlreichen Beispielen klinische Parameter und Merkmale zur Früherkennung und Prävention von Dysgnathien besprochen, Planungs- und therapeutische Möglichkeiten aufgezeigt, aber auch betrachtet, wo die Grenzen konservativer Maßnahmen liegen.

Tinkering with TADs!
Unique Mechanics with TADs to Achieve Complex Clinical Results

As dentists and orthodontists, we are inherently tinkerers. Since the inception of TAD mechanics, orthodontists have been tinkering with skeletal anchorage to achieve results not attainable with orthodontic therapy alone, or to achieve more efficient treatment than with only brackets and wires. As TAD technique armamentarium has expanded, the complexities of cases that can be treated and cases treatable without orthographic surgery have dramatically increased. This lecture will introduce attendees to a tinkerer’s handbook of unique TAD mechanics to treat extremely challenging cases.

The use of skeletal anchorage for open bite correction, the correction of severe crossbites in non-growing patients and difficult Class II and Class III mechanics with TADs will be presented. Techniques for correcting cases previously only fully treatable with orthographic surgery will be demonstrated. Techniques for palatal expansion with bone-borne skeletal expanders and their benefits will also be presented. Comparisons of skeletal only palatal expanders and tooth borne TAD expanders will be shown. Results and long-term outcomes of surgical treatment and relapse and TAD treatment and relapse will also be compared.
Retention modalities including techniques for using TADs with retainers to maintain open bite correction will also be shown.

The outcome of this lecture will be to not only show very unique treatments of challenging cases, but also work to inspire attendees to tinker with TAD treatment modalities to improve the outcomes of complex cases.

Temporary Teeth & TADs: TADs with Pontics in Growing Patients

The management of orthodontic treatment and retention of patients with missing teeth is one of the more complex modalities in orthodontics. This is exacerbated when treating and managing retention in growing patients waiting on cessation of growth prior to the placement of endosseous implants. When in the anterior segment, this is especially challenging. Techniques for the use of TADs and pontics to maintain space for future implants both during treatment and in retention will be presented. Case selection, appropriate TAD placement protocols and location, crown fabrication, & long-term management will be discussed. Bone biology in future implant sites and the management of bone and implant sites will be shown. Aspects such as changes during growth will also be discussed. New techniques integrating digital technologies in crown fabrication and TAD site selection will also be demonstrated. Treatment for both congenitally missing teeth as well as traumatically avulsed teeth will be presented.  Success rates and clinical guidelines from 15 years of TADs with Pontic teeth placed by the presenter will be shown.
